Pathway to Care / Understanding the care process

Understanding the
care process

You don’t have to navigate this alone.

The challenge

When care decisions can’t wait

Care needs rarely arrive at a convenient moment. For many families in Jersey, the need for support begins suddenly – after a fall, a hospital admission, or a gradual decline that can no longer be managed at home.

At a time when clarity is needed most, families are often left to navigate a complex system alone, uncertain of the next step, unsure of their options and anxious about the costs involved.

Pathway to Care exists to change that, providing a clear, direct route from first concern through to care in place.

The challenge

How care is usually arranged

Understanding the care system and avoiding delays.

For many families, the journey into care follows a familiar and often overwhelming path. Ultimately a referral is usually made and a Social Worker carries out a formal assessment to determine care needs and eligibility for Long-Term Care funding.

Your Social Worker assessment

Up to six weeks

Your Social Worker assessment can take up to six weeks to complete.

During periods of high demand, this assessment process can take up to six weeks. While families wait, they are often left wondering how best to support their loved one and what steps to take next.

This is where My Home Care can help

We provide an independent care assessment, practical guidance and clear information to help families understand their options.

Where appropriate, we can arrange care to begin promptly while supporting you through the Long-Term Care process, ensuring you receive the help you need without unnecessary uncertainty or avoidable delays.

Financial impact

Getting your assessment in place promptly

One of the most important steps you can take, it ensures your LTC funding begins from the right date and avoids unnecessary gaps in support.

It is important to know that after a Social Worker submits your application for Long-Term Care, you can wait for up to 12 weeks to find out the exact support you will receive. This can be a cause of financial stress and worry for you and your family.

My Home Care works with you and alongside the existing system to help ensure this process moves as smoothly and quickly as possible.
